Recreation Policies


Below you will find the policies of the Spanish Fork Parks & Recreation Department.
 

Liability Waiver
If you or any of your family members will be participating in any Parks & Recreation programs, you must have a current Lifetime Family Liability Waiver on file with the Parks & Recreation Office.


Refund Policy

This policy outlines the circumstances under which you might be entitled to a refund (full or partial) for an activity you have registered for.

 
Non-Resident Fee Policy

A Non-Resident is any person who physically lives outside of the Spanish Fork City Limits. An additional Non-Resident Fee is charged for leagues and programs.

By registering for Spanish Fork Recreation activities, you acknowledge that you are aware of the applicable code of conduct policies and agree to adhere to the guidelines outlined in these policies.

Concussion Policy
By registering for Spanish Fork Recreation activities, you acknowledge that you are aware of the Concussion Policy and agree to adhere to the guidelines outlined in this policy.

Volunteer Coaching Guidelines

Prospective coaches & assistant coaches for youth sports must read the Coach Code of Conduct and fill out the Right of Access Provider Waiver (which allows the city to run a background check). New background checks are required every three years.

Prospective Individuals should:

  • Read the Coach Code of Conduct carefully.
  • Fill out the Right of Access Provider Waiver completely, but do not sign until it is returned to Spanish Fork Parks & Recreation Office or personnel.
  • Return the waiver to the Spanish Fork Parks and Recreation Office (775 North Main Street) or to Recreation Personnel at the coaches' meeting.
  • FORMS CANNOT BE MAILED
  • Right of Access Provider Waiver must be signed at the time it is turned in so the signature can be verified.
  • Picture ID must be verified at the time the application is accepted

Employee Background Checks

All Parks and Recreation Employees, 18 years of age or older, must fill out a Right of Access Provider Waiver (which allows the city to run a background check) as part of the new hire process. This includes, but is not limited to, officials, referees, umpires, scorekeepers, instructors, facilitators, and supervisors.

Prospective Individuals should:

  • Fill out the Right of Access Provider Waiver completely, but do not sign until it is returned to Spanish Fork Parks & Recreation Office or personnel.
  • Return the ROA form to the Spanish Fork Parks and Recreation Office (775 North Main Street) or to Recreation Personnel at the employee training meeting.
  • FORMS CANNOT BE MAILED
  • Right of Access Provider Waiver must be signed at the time it is turned in so the signature can be verified.
  • Picture ID must be verified at the time the application is accepted
